About the File of Life Program

What is it?
Identification used by emergency medical services and law enforcement agencies to obtain vital medical and personal information in an emergency. It consists of: 1.) File-magnet with plastic sleeve on the front. 2.) Client data- a properly completed form.

Who is it for?
Everyone! Even if you do not have any health problems, list your family doctor, the hospital you prefer to go to (should a problem arise), and who you want us to contact in case of an emergency.

Who Distributes it?
A representative of Med-Act can provide you with a file for your home upon request. Anyone desiring to install the file in their home may obtain a kit from Johnson County Med-Act.

Where should it be placed?
On the front of your refrigerator.

Please consider the plight of the elderly citizen who lives alone, usually not by choice, but because family is gone, friends have moved, or resources are limited. In fact, anyone who lives alone shares the same concern, “What if I need help, call 9-1-1, and when help arrives, I am not able to communicate information vital to my survival?” The FILE OF LIFE PROGRAM is designed to give that reassurance.
